We document every decision in a google doc. In what we call a Decision Memo. Sometimes via Micro Decision Memos if it’s something minor.
It’s a tad painful once you get started (because it forces you to be very clear in describing problems and solutions) but we learned to type fast and furious, document things and always have references across departments on historical decisions

Anyone can open a decision memo for any part of our our organization. We encourage and we appreciate the initiative to fix things. It’s part of our culture (more about our quirky culture here)
And anyone can contribute with options. It’s an inclusive process as any individual can raise an objection on equal footing. Quality of reasoning outweighs rank
We are a fully-transparent org with the exception of information that the People Team deems confidential. Example of exceptions include: exact compensation of each employee, behavioral issues/plans, performance issues/plans, contents of private coaching conversations held by the Talent Development team
